Before you begin your application, you must obtain verification of your tuberculosis infection status from a medical professional. This is a new requirement for the 2026-27 school year under state law. (Current employees do not need to repeat their screening. Please check the box at the bottom of the page if you are a current employee.)
1. Download and review the the screening and certificate document for use with your medical provider.
2. Complete the Tuberculosis Risk Assessment Questionnaire found on the first page of the document.
3. Provide your questionnaire to a medical provider of your choice. Ask the provider to complete the "Certificate of Completion Tuberculosis Risk Assessment and/or Examination" found on the second page of the document.
3. Upload the completed Certificate of Completion using the box below. Only the Certificate of Completion must be uploaded, you do not need to submit your responses to the questionaire.
Alternatively, you may also ask your health provider to provide a written letter stating that you have been examined and determined to be free of infectious tuberculosis.
Please note: Volunteer applications submitted without a valid Certificate of Completion or appropriate documentation from a medical professional will not be accepted and may require that you restart the volunteer application process.
